To create your chaos weapon just go to NPC Chaos Globin in Noria and select the 4th option
For this combination you will use
- 1x Jewel of Chaos
- 1x Normal or Exc Item at least +7 +4 (additional life)
If you want to increase the chance of success, you can add a Jewel of Bless or Soul to the combination.
If your combination succeeds you will receive a random chaos weapon, if you fail you will lose your gems.
